The legal description of the 6.702-acre property is Lot 17, Lazy O Ranch PUD. The permit and record drawing was provided by Pitkin County Environmental Health Department (Parcel ID #2645-091-01-017). The subject OWTS consists of one 1500-gallon, single-compartment concrete septic tank followed by a 1000-gallon, two-compartment septic tank with an Orenco® pumping system in the second compartment. Effluent is pumped to an automatic distributing valve (ADV) and then to six low pressure drip dispersal zones, each with three 8-foot by 85-foot sections. Individual Sewage Disposal System Permit 08041 documents this system. The system was sized to accommodate 6 bedrooms (5 bedrooms in the main house + 1 bedroom in a detached ADU). The permit did not receive final approval; however, a partial final letter was received from the design engineer dated September 20, 2010. The inspection was conducted early in the development of the property and there was no electricity for the pumping system at the time of their inspection. Pitkin County also details notes from an inspection conducted by staff in September 14, 2010. A clean out is located on the sewer pipe before the septic tanks. The septic tanks are located to the west of the residence. All manhole lids to the septic tanks and ADV were accessible from grade. The inlet tee entering the first septic tank was cocked upward indicating settling of the pipe outside of the septic tank. We recommended this be corrected and Larry Martin Construction excavated and replaced the pipe between the clean out and the first septic tank. The septic tanks were pumped by All Valley Resource at the time of our inspection. We tested the pump and alarm and each were functioning properly. We observed the operation of the ADV and it appeared to be working properly. The filter was clean at the time of our inspection. There is access to all valves on each low pressure drip line section. Some of the valve boxes had been filled with dirt and these were cleaned out by Larry Martin Construction. They also made a key for the valves and left it in the uppermost valve box. All zones are currently ON and should remain on because of the ADV. Inspection ports are present at the end of each section. All ports were dry. We walked the soil treatment area (STA) and found evidence of surface saturation or failure at the time of our inspection. This evaluation is not a guarantee of future system performance. This inspection is good for one year. Recommendations: • Clean effluent filter annually, or as needed. This report and a completed OWTS Use Permit Application must be submitted to the Pitkin County Environmental Health Department. Changes must be approved by this Department and the engineer prior to construction. This design is for a low pressure drip system due to soil percolation rates ranging from 120 to 240 mpi*. Design is for one 1500 gallon one -compartment concrete septic tank followed by a 1000 gallon two- compartment concrete tank with an effluent pump in a Biotube pump vault. An automatic distributing valve distributes to a low pressure drip field which is to be installed in eighteen 8 -foot by 85 -foot sections, six zones with three sections per zone. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. COWC prepared OWS design documents under Job No. C479 dated June 29, 2007 which was revised April 8, 2008. A site visit was performed on September 10, 2010, at the request of Scott McCurry, of M & M Excavation, Inc, the contractor of record. At the time of the site visit, a 1500 -gallon single compartment and 1000 -gallon two-compartment GJ Pipe, concrete septic tanks were installed with inlet tees and an Orenco biotube pump vault in the second compartment of the second tank. Two- inch Schedule 80 PVC was installed from the pump to the 6 -outlet automatic distributing valve ADV). Two-inch Schedule 80 pipe was installed from the ADV to the manifolds for the drainfields. The manifolds were level and contained 2 -inch ball valves. The drainfield was installed in six zones with three sections each. V The well was not installed at the time of the site visit. COWC shall be called for an additional observation once the well is installed and power is available and the OWS is connected and functioning. It is our understanding that the contractor is storing the control panel for the system until it is ready for installation. COWC must see the functioning control panel at the time of our final site visit. The observed components of the OWS appear to be installed in general conformance with the CHURCH OWS design, plans and specifications. This letter provides information from observations made during one visit to the subject site by COWC. COWC observed installed components of the OWS. COWC did not observe the installer's methods or the equipment selected by the installer and/or client.
