ASPEN VALLEY MEDICAL FOUNDATION
GRANT CONTRACT

GRANTED: $32,000
PURPOSE: 50% matching grant to equip patrol cars with twenty-one (21)
Life Pak 500 Automated External Defibrillators
GENERAL CONDITIONS
Upon acceptance of this grant, Grantee agrees to following terms. Should these terms not
be adhered to, Grantee will be expected to return all grant monies to Aspen Valley Medical
Foundation:
1. Grantee agrees that this grant is to be used for the specific purpose(s) listed above.
Grantee agrees these funds may be expended for no other purpose without the expressed,
written approval of the Aspen Valley Medical Foundation. Grant Funds are to be applied as set
forth in the budget specified in Grantee's Grant Application.
Funds disbursement will be agreed upon by Grantee and Grantor.
2. Grantee agrees that no part of the funds received from this grant will be used to carry on
propaganda, influence legislation, influence the outcome of any specific public election, or to
carry on, directly or indirectly, any voter registration drive.
3. Grantee agrees to immediately notify the Aspen Valley Medical Foundation in writing if
Grantee has reason to believe that the grant monies cannot be expended for the specified purposes
listed above.
4. Grantee understands that Aspen Valley Medical Foundation considers each grant request
on an individual basis, and that this grant is not to be construed as establishing a precedent for
further support of Grantee.
5. Grantee agrees to submit a Progress Report by September 18, 1999 which will include
quantitativeand financial information about the progress of the project. A Final Report shall be
due by December 18, 1999, or prior to that, at the conclusion of the expenditure of total
Grantor's funds, whichever occurs first. The Final Report shall summarize accomplishments and
should include a financial report for the project, If Grantee is unable to expend funds by

March 18, 2000, Grantee will be required to provide written explanation to Aspen Valley
Medical Foundation.
6. With respect to the specific purpose(s) for which this grant has been made, Grantee
agrees to promote Aspen Valley Medical Foundation in any and all media released to the
public. Proposed media releases shall be submitted to the Aspen Valley Medical
Foundation for approval prior to release. Grantee agrees to promptly furnish the Aspen Valley
Medical Foundation with copies of information regarding all news releases, published materials,_
and/or any mention of this grant in a public media which comes to Grantee's notice or attention.
Grantee agrees to acknowledge the funding support of Aspen Valley Medical Foundation,
with respect to the specific purpose(s) monies have been granted for, in all printed
materials and announcements concerning this project. Aspen Valley Medical Foundation
asserts its right to release information regarding this grant to any public media.
7. Grantee shall cooperate with Aspen Valley Medical Foundation in supplying additional
information or in complying with any procedures which might be required by any governmental
agency in order for Aspen Valley Medical Foundation to establish the act that it has observed all
requirements of the law with respect to this grant.
THE UNDERSIGNED, on behalf of Pitldn Copnty Sheriff's Office, agrees to the terms and
conditions of the grant described herein.
