In accordance with Paragraph 10 of the contract with routing number 9807588 and contract number FAA, EMS 9807588 , hereinafter
referred to as the Original Contract (copy attached and by this reference made a part hereof) between the State of Colorado, Department
of Public Health and Environmejit EMSP Division and Pitkin County, the parties hereby agree the Original Contract is hereby renewed
for the period of January 1, 199rthrough December 30, 1999. The parties agree that the maximum amount payable by the State for
the eligible services during this renewal period is FIFTEEN THOUSAND NINETY TWO DOLLARS AND THIRTY THREE CENTS
($15,092.33)

According to Paragraph M i0 of contract FAACPD960228. Routing Number 960228 (copy attached and by this
reference made a part hereof) between thc State of Colorado. Department of Public Hcaltll and Environment and
Pitkin Board of County Commissioners. the State will cause to be paid to thc Contractor the sum of twenty-four
thousand. six hundred %wenty six dollars and seventy-three cents (S24,626.73) to bc reimbursed monthly based
on a sum equal to 1/12 of the contract amount upon receipt of signed affidavits or expenditures beginning July

Payment pursuant to this contract will be made as earned, in whole or in pan. from available State funds
encumbered in an amount not to exceed two hundred and thirteen thousand. three hundred and twelve dollars
(S213.312) for thc purchase of County Environmental Health Scrviccs. This amount was determined using the
1997 Long [fill appropriation under Local Health Services for Public Health Sanitarians in areas not served by
local and regional health departments. The terms and conditions of the original contract shall remain the same.
