PROJECT NAME: CEMETERY LANE PAVING & SLAUGHTERHOUSE BRIDGE REHAB PROJECT

The Master Service Agreement for Provision of On-Call Engineering Services (the “Agreement”) dated
February 1, 2025 between the Board of County Commissioners of Pitkin County (the “County”) and SGM,
Inc 118 W Sixth St, Suite 200 Glenwood Springs, CO 81601 (the “Contractor”), shall include the following
services.
1. Contractor’s Obligations. Contractor shall provide a construction survey and materials testing for the
Cemetery Lane Paving & Slaughterhouse Bridge Rehab Project as described in the Contractor’s Proposal
(“Attachment A”).
2. Compensation and Expenses, Invoicing, Payment and Offset. The County shall compensate
Contractor for its services in accordance with the Project Budget and Schedule set out in Paragraph 1
of this Task Order. It is expressly understood and agreed that in no event will the total compensation
and reimbursement to be paid hereunder exceed the sum of Twenty Three Thousand dollars and Zero
cents ($23,000.00) for all services rendered. By Task Order or Task Order Amendment, the County
and Contractor may reallocate the budget among project tasks if the total budget amount remains
unchanged. Contractor shall invoice for the project monthly based on hours worked, with payment
expected within thirty (30) days of invoice, but any payment by the County may be offset by any
amount the Contractor owes the County for any reason.

Any invoices not sent in the following manner may have payment delayed. All invoices for this Task
Order shall reference Task Order 012.25 B-5 and Cemetery Lane Paving & Slaughterhouse Bridge Rehab
Project . Invoices shall be sent electronically in PDF format to romany.iacono@pitkincounty.com.
3. Pitkin County’s Obligations. Pitkin County shall administer this contract through a County
Representative. Chris Baroody , Project Engineer will manage the project as the County’s
Representative. The services provided and products delivered by the Contractor under this contract
will be subject to review by the County’s Representatives, or a designee, for compliance with
Contractor’s obligations prior to final payment.
4. Formation of Task Order. This Task Order is issued in accordance with the provisions of the
Agreement. Contractor agrees to provide services subject to the terms of this Task Order and for the
avoidance of doubt this Task Order consists of the terms set out in the Agreement. In all other respects
the Agreement is in full force and effect and remains unchanged by this Task Order.

1. Contractor’s Obligations.
A. Project Description
This project construction survey staking and materials testing for the Cemetery Lane Paving
and Slaughterhouse Bridge Rehab Project.
B. Work Plan
Task 1 – Project Management and Task Order Administration
This consists of ongoing project management and coordination between Pitkin County and
the consultant team.
Task 2 – Construction Staking
Construction staking to include setup of control used to generate topo survey performed by
SGM in May of 2025 and as many of four separate stakings, including:
1. Initial staking of finished grade of bridge deck and approach paving after removal of
HMA surfacing of bridge. Staking should include staking of centerline and EOP every
10-feet on bridge and 150-feet on either side of bridge.
2. The grade and elevations of bridge deck will be verified and the finished grade and
profile may need to be adjusted to limit depth of paving on bridge. SGM may be
required to restake centerline and EOP on bridge. (to be determined if needed)
3. Staking of curb profiles on east side of bridge at north and south approaches as well as
staking of grading widening for guardrail end treatments
4. As-built survey of bridge deck and approaches (to be determined if needed)
Task 3 – Materials Testing (GROUND Engineering)
• Materials testing should include two days for testing and sampling with breaks of Class
D concrete used for curb and gutter and sidewalk

•An estimate of five days for testing and sampling with breaks for Class DR concrete
used for bridge deck repairs.
•One day of sampling and densities of HMA overlay paving of Cemetery Lane.
C. Deliverables
•Materials testing reports (through GROUND Engineering document portal)
D. Schedule
•Construction is anticipated to begin the first week of April and continue through the
Summer.
E. Exclusions and Limitations
•Utility locates are not included
2. Compensation and Expenses.
Total compensation and reimbursement to be paid hereunder shall not exceed the sum of:
Twenty-Three Thousand Dollars ($23,00.00) for all services rendered. Fees estimated by
task are as follows:

Getting paper or electronic copies
