The Provision of HCPF Medicaid Grant Consultant (the “Contract”) dated August 27, 2025
between the Board of County Commissioners of Pitkin County, Colorado (the “County”) and
EW8 Productions (the “Contractor”), is hereby amended as follows:
Description of Change:
1. Incorporation of additional Contractor Scope of Work (“Attachment A”)
2. Allocation of funding in an amount not to exceed $25,000.00 for services provided,
billable only as incurred.
3. Extension of the contract term through October 31, 2026.
Reason for Change: To accommodate additional scope of work identified during the initial
contract term and to provide funding for the associated Human Services Support Consultant
services required to support policy implementation, compliance, and operational improvements.

ATTACHMENT A
SOW Extension of Human Services Support Consultant
This SOW is for services provided July 1, 2026 – Oct. 31, 2026 as an extension of support
provided under the SOW Human Services Audit Management and Interim Operational
Support by EW8 Productions, LLC.
During the first SOW period, in preparation for the Management Evaluations by HCPF and
CDHS, it was identified that Pitkin County Economic Assistance Division did not have
policies or procedures in place for their Medicaid or SNAP programs. In order to comply
with State requirements, 25+ procedures for Medicaid, 3 policies for Medicaid and 12+
policies and procedures for SNAP, which would bring the county into compliance, were
written, in collaboration with County staff members. Additionally, the work will include
cross referencing Economic Assistance Division policies with all other Department
policies to ensure alignment and cross-referencing.
During this time the need was identified for additional support by EW8 Productions, LLC for
the Economic Assistance Division for the following:
1. Thorough documentation of process improvements and compliance measures,
including creation and institutionalization of a Pitkin County Economic Assistance
Manual. $12,500
2. Supervisory and operational support activities including a written Action Plan for the
Division’s implementation of policies and procedures into daily, weekly, monthly,
quarterly and annual business practices. $7,500
3. Management coaching and change management support for strengthening
compliance and operational practices. $5,000

Getting paper or electronic copies

Consequences of changing your mind

Unless you tell us otherwise in accordance with the procedures described herein, we will provide

To request paper or electronic copies from Pitkin County
To request delivery from us of paper or electronic copies of the notices and disclosures
previously provided by us to you electronically, you should request those documents from Pitkin
County under the Colorado Open Records Act by contacting the Pitkin County custodian who
sent you the document for signature.
To withdraw your consent with Pitkin County
To inform us that you no longer want to receive future notices and disclosures in electronic
format you may:
i. decline to sign a document from within your DocuSign account, and on the subsequent
page, select the check-box indicating you wish to withdraw your consent, or you may;
ii. send us an e-mail to Helpdesk@provelocity.com and in the body of such a request, you
must state your e-mail, full name, Postal Address, telephone number, and account
