WIIEREAS. authority exists in the Lev and Funds have been badgered, appropriated and othenvise made
available and, a sufficient tmencumbered balance thereof remains available for payment in Fund Nnmber
G/L Accom~t Number. 55357 , Contract Encumbrance Number. C291098 .; and
WHEREAS, required approval, clearance and coordination has been accotnpl[sbed from and witix' appropriate
agencies: and
WHEREAS,'a with respect to community sanitation services, the State in order
to carry out its lawful powers, duties and functions pursuant to Article 3,
Chapter 66, Colorado Revised Statutes, 1963, as amended, has determined that the
services of one full-time Public Health Sanitarian (and one Sanitarian
Technician) are required in Pitkin County ; and
WHEREAS, the Contractor is able to provide community sanitation services.
NOW THEREFORE, itishereby a~eedthat
1. '4 Th~ Contractor shall provide, through a Public Health Sanitaria?,
community sanitation services, and in consideration of lthese services the Stat~ will
cause to be paid to the Contractor a sum not to exceed Six Thousand Three Hundred
Twenty and no/100 .................. dollars ($ 6,320.00 ) as follows:
(a)
Reimbursement will be made monthly, based on a sum equal to one-fourth
of salary, retirement and/or so.cial security paid by the Contractor
for a public health sanitariaa (and sanitarian technician); upon
receipt of affidavits of expenditures submitted in duplicate on fo~ms
supplied by the State.
2. Prior to emplosnnent of a public health sanitarian (and sanitarium
technician) the Contractor will submit Application for Employment to the
Sanitation Section for review and approval, based upon Recon~ended Personael
Standards. On or after the date of this contract the Contractor will etlploy,
under the contract only sanitarians who are so approved.
3. The State will continue payment for sanitation personnel only so long
as the sanitarian renders acceptable services as determined by the State, and the
State shall have the authority for sanitation administration, snpervision, and
periodic evalnation o~ the quality and quantity of service.
4. The term of this contract shall be Jnly 1, 1974, through Jmie 30, 1975.
5. Thin contract may be cancelled and ter,-minated by either party hereto,
but the party desiring to ter~ninate or cancel must give written notice of their
intention thirty (30) days prior to the date of cancellation.
