Upon approval of the application, the access approach shall be constructed, maintained and used in accordance with the Pitkin County Asset Management Plan. Construction of access shall meet all County road access standazds. 3. The County may revoke the permit if at any time the permitted access approach and its use violates any of the terms and conditions of this permit. 4. Work under this permit applicatioh will consist of constructing a temporary driveway for the purpose of accessing a well site on her 35+-acre parcel above Brush Creek Village on Medicine Bow Road. LOCATION: Attached as EXHIBIT "A" is a vicinity map of the property. The property is located above the Brush Creek Village Subdivision south of Upper Medicine Bow Road. LAND USE APPROVALS: The parcel has received the necessary land use approvals for the work proposed. Attached as EXHIBIT "B" are copies of the land use approvals that are in place. Ms. Cesery has recently gained approval from the Hearing Officer for reinstatement of the 1041 Hazard Review for the existing building envelope and has gained approval for the site to receive up to 2 TDRs to increase density. DESCRIPTION OF THE WORK: The project entails the construction of a primitive access road through the "Barn, Access, Landscape and Leach Field Envelope" and into the Building Envelope. The road will be used this fall to access a preferred well site located within the Building Envelope. The road will be approximately 750-feet long and will have an averagegrade of about 8.5%. Actual construction activity will consist of clearing and grubbing a 15-foot wide path along the road alignment; cutting and filling to place a flat road bed of no more than 10 to 12-feet in width; moderate compaction of the native materials. along the road bed; and placement of a drainage culvert at the intersection of the access road. with Medicine Bow Road. The culvert will conform to County Standards as shown in this application. Attached as EXHIBIT "C" is a plan and profile for the road. Silt fencing will be placed along the downhill side of the road near the intersection with Medicine Bow Road to insure that drainage from the work does not impact the road7ight- of-way. Because the road is temporary in nature, no r•evegetation will be conducted at this time. However, we will be coming back ih to you next spring with an Access Driveway Development Permit and Earthmoving Permit to placeLtilities`in the road bed and to construct a more permanent driveway along this alignment. A revegetation plan and drainage plan that conforms to County standards will be submitted with this future permit application. CONSTRUCTION SCHEDULE: We are ready to start the project as soon as the permit has been issued. The work is scheduled to be completed by December 15, 2005.
